Liability Waiver, Express Assumption of Risk, and Release of Liability
Conscious Mommy, LLC provides an online and in-person forum for discussion where parents can share thoughts, ideas, and concerns related to their parenting experiences. The classes and coaching services provided by Conscious Mommy, LLC are led by a licensed mental health professional; however, these services are not intended to be a substitute for the advice and/or treatment by your health care providers; nor are these services to be received as mental health treatment. Furthermore, the needs of each child and parent are unique, and no service or forum provided by Conscious Mommy, LLC is intended to replace the advice of a healthcare and/or childcare professional.
“Conscious Mommy Circles” are held in a rented facility. Despite the high standard of care and level of attention to detail to ensure safety standards, it still remains possible that during your participation in any of our classes, you may be exposed to a variety of unforeseen hazards and risks. These risks include, but are not limited to serious personal injury and property damage (“Injuries and Damages”) caused by the condition of the rented premises. You understand and acknowledge that you may have to exercise extra care for yourself. You further understand that Conscious Mommy, LLC does not have rescue or medical facilities or the expertise necessary to deal with any Injuries and Damages.
You further confirm that:
- You are the parent and/or lawful guardian of the child.
- ACTING ON BEHALF OF YOURSELF, AND TO THE EXTENT ALLOWED BY LAW, YOU HEREBY WAIVE AND AGREE TO RELEASE ANY CLAIMS OR LIABILITY WHICH YOU, YOUR CHILD, OR YOUR CHILD’S HEIRS AND SUCCESSORS MAY HAVE OR BELIEVE TO HAVE AGAINST Conscious Mommy, LLC AND ITS OFFICERS, EMPLOYEES, OR AGENTS FOR ANY INJURIES, LOSSES OR DAMAGES TO YOUR CHILD, YOUR CHILD’S PERSONAL PROPERTY AND YOUR PERSONAL PROPERTY.
- YOU AGREE TO BE RESPONSIBLE FOR, INDEMNIFY, AND HOLD HARMLESS Conscious Mommy, LLC FROM AND AGAINST ANY CLAIMS, SUITS, JUDGMENTS, OR COSTS WHICH MAY BE BROUGHT AGAINST Conscious Mommy, LLC, ITS OFFICERS, DIRECTORS, EMPLOYEES OR AGENTS FOR THE ACTUAL OR ALLEGED ACTS OR OMISSIONS OF YOU OR YOUR CHILD.
I have read the above Liability Waiver, Express Assumption of Risk, and Release of Liability in its entirety and I freely and voluntarily assume all risks of such Injuries and Damages and notwithstanding such risks, I agree to participate in the services and workshops provided by Conscious Mommy, LLC.
